Diabetes Prevention Program

A Self-Management Program to help you get more active, lose weight, and decrease your risk of getting diabetes. This Nationally recognized evidence based program can provide peer support and tools to help you adopt healthier eating and physical activity habits that can lead to weight loss and reduce your risk of getting type 2 diabetes. You have a greater risk of getting diabetes if you:
- are overweight or obese
- don’t get enough physical activity
- have high blood pressure
- have abnormal cholesterol
- have a parent or sibling with diabetes
Participants meet for one full year, one hour once a week for 16 weeks, then every other week for 8 weeks and then once a month for the last 6 months.
